I'm having the identical issue. Has anybody figured this out? Is it a z-index issue in the CSS?

Added my image. Same issue. Visitors can't answer Yes/No on survey.

In order not to have any conflicts between flash objects and the DHTML layer you need to set the Flash object property to param NAME=wmode VALUE=transparent (between brackets)
